A review record and queue for AI-drafted clinical notes
01In late May 2026 Sweden’s data protection authority spent two days at Region Gävleborg, which runs an AI scribe across 26 primary care centres.
A clinician edits each draft for a median of 93 seconds, approves it, and the audio is deleted. The inspectors asked how the region finds and corrects transcription errors, and a disputed note had only a spreadsheet of re-reads behind it.
02Corrigenda is software: a review record for every note the scribe drafts, and a review queue worked by a clinician the centre names from its own staff.
A browser extension saves the first draft and the approved text once, at approval, and never edits them. Each night the approved notes are scored, and the flagged ones, plus a 2% random sample, land in that reviewer’s queue, due five working days later.

Rules for sound-alike Swedish drug names, doses, left and right, and dates find about a third of the errors reviewers confirm. The rest falls to a Swedish-language model fine-tuned per region on its reviewers’ verdicts. The second classifier sorts edits into transcription fixes, clinical rewording and formatting.
04A passage scoring above 0.7 goes to the centre reviewer’s queue; below that, the note is seen only if the random sample picks it.
The verdict is the reviewer’s and the correction is the author’s, made in the journal under its own rules, and nobody at Corrigenda reads the note. Each verdict is kept with its passage, identifiers removed, never pooled with another region’s, and trains that region’s own model.
05Onboarding a region into Corrigenda means scoring its last twelve months of notes within a week, against a few thousand on an ordinary night.
Patient notes may not leave Sweden or pass through another company’s model API, so inference and fine-tuning run on cloud GPU capacity we control in Stockholm. From Q1 2027 the edit classifier is a fine-tuned open-weight Swedish model there, and by Q3 2027 we plan to run backfills on GPU capacity booked in advance.
06Corrigenda AB is a Swedish aktiebolag in Gothenburg, registered in November 2025, and the software opened in September 2026.
A centre pays EUR 16,800 a year, and the first centre runs sixty days before it is invoiced.
07Corrigenda has issued no invoice yet, because no centre has reached its sixtieth day.
We will not name the care providers we work with or write case studies about them; one under supervision has good reason to keep quiet, and we keep quiet with it.
